"Embarking on a Journey of Self-Discovery Can Raise Many Questions and Professional Guidance Can Help You to Uncover the Answers."
WHAT DOES 'LCSW' STAND FOR?
The credential that appears at the end of my name stands for Licensed Clinical Social Worker. In New York State, it means I have completed an accredited post-graduate program at a college or university, passed the requirements of the State Education Department for licensing, and an additional exam for clinical expertise. Anyone can verify my credentials by going to www.op.nysed.gov/opsearches.htm. A Licensed Clinical Social Worker is approved to practice independently, counseling individuals, families, or couples, depending upon his/her training and experience, and may be reimbursed by some health insurance companies.
HOW DO I SCHEDULE AN APPOINTMENT?
I find that it is most helpful to discuss one's treatment needs over the phone rather than through text/email, which are not secure and confidential. I offer a free telephone consultation, however, if you leave a voice message, it is important to include a time of day to reach you that would allow for quiet and privacy to discuss the reason(s) for seeking treatment. You can reach me at 631-836-3027. All voice messages are confidential. I OFFER VIRTUAL & INPERSON SESSIONS
WHAT SHOULD I EXPECT AT THE FIRST APPOINTMENT?
The issues that bring you to the appointment may be multiple and complex. It may even be the first time you are sharing a complete description of what you are struggling with which can feel overwhelming. In order to obtain the information that will allow me to be most helpful to you, I ask specific questions that relate to your family and medical history as well as prior mental health treatment. I want to hear your story, however, it is important not to feel pressured or rushed to cover every detail. If you decide to work with me, whatever information needs to 'come up' will do so naturally as your treatment progresses.
WHAT PAPERWORK WILL I NEED TO COMPLETE?
At the initial appointment, I ask that you complete a personal information fact sheet with your contact info, referral source, and primary care physician. There are some additional forms to read and sign regarding privacy as well as a description of my cancellation policy. These forms can be read and signed electronically for your convenience. I welcome any questions/concerns pertaining to these forms and will address them in the session.
WHAT ARE THE FEES?
My goal in fee-setting is to help you feel comfortable discussing financial issues and to come to an agreement that prevents the premature termination or disruption of treatment due to cost. Fees are adjusted over the course of treatment and may be increased or decreased depending upon changes in your circumstances.
